Apparatus and method for treating a fracture of a bone of a subject includes inserting an intramedullary (IM) elongated member, such as a nail, in a medullary canal of the bone of the subject. The elongated (IM) member has a proximal head that defines at least one hole therethrough. A sleeve, which includes a locking mechanism, is provided to engage the hole when the sleeve is inserted in the hole, such engagement preventing rotational and longitudinal movement between the sleeve and the hole. The system may include a screw, the sleeve being adapted to slidably receive the screw. An extension member is provided to extend the length of the elongated (IM) member to a desired length. A fixation member may be attached to the elongated (IM) member to fix and retain a broken lateral wall of a fractured bone.
